L-Vit 12 Vial contains cyanocobalamin, a synthetic form of vitamin B₁₂, typically at a strength of 1,000 µg/mL.
It is used as an injectable vitamin, either intramuscularly (IM) or subcutaneously (SC).
Vitamin B₁₂ is essential for red blood cell formation, DNA synthesis, nerve function, and energy metabolism.
Vitamin B₁₂ deficiency
Megaloblastic anemia (commonly due to poor absorption or pernicious anemia)
Peripheral neuropathy caused by B₁₂ deficiency
Fatigue, weakness, and memory issues linked to B₁₂ deficiency
Diagnostic use to evaluate vitamin B₁₂ absorption in certain conditions
Cyanocobalamin is converted in the body to active forms of vitamin B₁₂.
It helps enzymes involved in:
Methionine synthesis, which supports nerve and brain function
Succinyl-CoA production, important in energy metabolism and red blood cell health
These pathways are crucial for healthy nerves, blood, and DNA integrity
Standard strength: 1,000 µg/mL per vial
General protocol:
Daily injections for 5–7 days
Then alternate days for 2 weeks
Then weekly for 4 weeks
Monthly maintenance dose thereafter, especially in chronic deficiency
Administered by a healthcare professional via IM or SC route
Dosage and frequency can vary based on severity and diagnosis
Not to be used in patients allergic to cobalt or cyanocobalamin
Avoid in patients with Leber’s optic atrophy due to risk of vision worsening
Use cautiously in premature infants or those with renal impairment due to preservatives (e.g., benzyl alcohol or aluminum)
Monitor for low potassium levels (hypokalemia) during rapid correction of anemia
Safe in pregnancy and lactation only if clearly needed
Generally well tolerated
Common mild effects:
Pain or redness at the injection site
Headache
Nausea
Mild diarrhea or dizziness
Rare serious reactions:
Allergic reaction (rash, swelling, breathing difficulty)
Rapid heartbeat
Fluid retention
Monitor complete blood count (CBC), vitamin B₁₂, folate, iron, and electrolytes
May require folic acid supplementation alongside
Long-term therapy may be lifelong in chronic malabsorption or post-gastrectomy patients
