Orlistat is a medicine used for weight management in overweight and obese adults.
It works locally in the stomach and small intestine to reduce the absorption of dietary fat.
Brand names: Xenical (120 mg), Alli (60 mg), and various generics.
Orlistat 120 mg capsules (hard gelatin form, usually blue or turquoise).
The 120 mg dose is the prescription strength used under medical supervision.
Orlistat inhibits gastrointestinal lipases (enzymes that break down fats).
As a result, about 25–30% of dietary fat eaten passes through undigested and is eliminated in the stool.
This reduces calorie absorption → helps in gradual weight loss.
Weight reduction in obese adults (BMI ≥30).
Overweight adults (BMI ≥27) with additional risk factors such as:
Type 2 diabetes
Hypertension
High cholesterol
Helps in maintaining weight loss after reduction.
May improve glycemic control in type 2 diabetes patients.
Usual dose: 120 mg capsule, three times daily.
Taken with meals containing fat (during or up to 1 hour after eating).
If a meal is missed or contains no fat → dose can be skipped.
Should be combined with:
Low-calorie, reduced-fat diet
Exercise program
Not recommended in:
Pregnancy or breastfeeding
Chronic malabsorption syndrome
Cholestasis (liver/bile flow disorder)
Must take a daily multivitamin (A, D, E, K, beta-carotene) at bedtime, as Orlistat reduces absorption of fat-soluble vitamins.
Use cautiously in diabetes (may need adjustment of anti-diabetic drugs).
Cyclosporine (reduced absorption; take 3 hours apart)
Warfarin (affects Vitamin K → INR monitoring needed)
Levothyroxine (reduced absorption; separate by 4 hours)
Some anti-seizure medications
Average weight loss: 5–10% of body weight in 6–12 months, when combined with diet + exercise.
Also helps improve cholesterol, blood sugar, and blood pressure.
