Xarelto is the brand name for Rivaroxaban, a direct oral anticoagulant (DOAC). The 10 mg tablet is typically used to prevent blood clots (deep vein thrombosis or DVT) especially after surgeries like knee or hip replacement.

Rivaroxaban works by:

• Inhibiting Factor Xa, an essential enzyme in the coagulation cascade

• This prevents the formation of thrombin, a key substance needed to form clots

• As a result, it helps prevent or treat blood clots in veins or arteries

Unlike older anticoagulants (like warfarin), Rivaroxaban works without requiring routine blood monitoring.

Note: For atrial fibrillation, DVT treatment, or stroke prevention, higher doses (15 mg or 20 mg) are typically used.

Take with or without food. For higher doses (15/20 mg), take with food.

• Other anticoagulants/antiplatelets (e.g. aspirin, clopidogrel) → increased bleeding risk

• NSAIDs (ibuprofen, diclofenac) → increased bleeding risk

• Rifampicin, phenytoin, carbamazepine → reduce rivaroxaban levels (less effective)

• Azole antifungals, ritonavir → increase rivaroxaban levels (risk of bleeding)

• Fixed dosing (no routine INR monitoring like warfarin)

• Rapid onset of action

• Convenient oral tablet

• Shorter half-life compared to warfarin

• Missed dose: Take as soon as possible the same day. Skip if it's almost time for the next dose.

• Overdose: Can lead to life-threatening bleeding—seek emergency care.

• Surgery: Must be stopped in advance to reduce bleeding risk—follow doctor’s guidance.
